**Job Title: Home Health Physical Therapist**

**Location:** Southborough, MA

**Job Description:**

We are seeking a Home Health Physical Therapist for a 13*week contract, Monday to Friday, 8:00 AM to 4:30 PM. Provide quality care to patients in their homes, manage cases, and collaborate with healthcare professionals.

**Key Responsibilities:**

* Deliver physical therapy services to home health patients.
* Complete documentation after each visit.
* Manage daily cases, including Start of Care and at least four visits.
* Collaborate with healthcare team for comprehensive care.

**Requirements:**

* Valid MA Physical Therapist License.
* Basic Life Support (BLS) certification.
* 2+ years of home health experience.
* Familiarity with EMR, especially Home Care Home Base, preferred.

**Additional Information:**

* No on*call or holiday duties.
* Possible floating to other locations if needed.
* COVID*19 vaccination not required.
