**Job Title: Home Health Physical Therapist**
**Location:** Walnut Creek, CA
**Job Description:**
Join our team as a Home Health Physical Therapist, focusing on geriatric care. This 13*week contract, with potential extension, runs Monday to Friday, 8:00 am to 5:30 pm.
**Responsibilities:**
* Provide quality physical therapy to home health patients.
* Manage a caseload of about 6 patients daily.
* Schedule and coordinate patient visits.
* Collaborate with RNs and OT/PT staff.
* Use EMR systems like Oasis and HCHB (training available).
**Requirements:**
* Valid CA PT license.
* BLS certification.
* At least 1 year of home health experience.
* Preferably experienced with geriatric patients.
**Additional Information:**
* No on*call or holiday duties.
* Flexible scrub color.
* Includes HR, HCHB training, and mentorship.
