**Position Details:**
* 13*week contract, 40 hours/week, Monday*Friday, some weekends
* Day shifts, flexible hours, no on*call
**Qualifications:**
* Active Physical Therapist License
* BLS Certification
* 2+ years of Physical Therapy experience
* Home Health experience required
* Familiarity with Home Health, HCHB, and Oasis systems
**Responsibilities:**
* Provide physical therapy services in patients’ homes
* Collaborate on individualized care plans
* Document progress using EMR
**Additional Information:**
